Health Requirements
The Hawaii School Attendance Law requires all students to meet the following before they attend any school in the state for the first time. A tuberculosis clearance (written proof of a negative skin test: Mantoux/PPD or chast x-ray completed within one-year of school entry), and an up-to-date immunization record which includes protection against diptheria, tetanus, pertussis, polio, measles, rubella, mumps, hepatitis B, and chicken pox. If some or all of these are not available upon entry, an appointment slip issued from the physician's office may be acceted as temporary indication of the family's good faith effort to comply with this requirement.
Important: By the first day of school, all new students to any public or private school in the State of Hawaii must have: A) tuberculosis )TB) clearance; B) a completed Student Health record (Form 14) including a physical examination and all required immunizations OR a signed statement or appointment card from your child's doctor.

Grade Reporting Procedure
The academic progress of each student is reported four times a year; approximately two weeks after each quarter ends through the report card. Progress reports are distributed to parents during the middle of each quarter.
Report cards are taken home by the student for the first three reporting periods. The final report card is mailed home in June.
Parents should call the school and contact their child's counselor if they fail to receive a report card. If they wish to schedule a conference to discuss their child's progress, parents should call and leave a message for the child's teacher(s). (808) 622-6525.
Transfer Procedure for a
student leaving Wheeler Middle School Spartans: parent or legal guardian must submit a completed Temporary/Permanent Release Form at least one (1) week before the child's last day in school. This will enable the office staff to secure the necessary information and prepare a release packet for the child.
Early release at the end of the school year: releases are processed as follows in accordance to the State School Code.
During the last four (4) weeks (final week may be a partial one): final grade given; promotion/retention indicated; enrollment at new school optional.
During fifth (5th) and sixth (6th) week before end of school: release (Quarter) grade given; promotion/retention recommended, based on state estimate of anticipated final grades at new school.
Prior to sixth (6th) week of the end of school: Normal release; no fourth (4th) quarter grade given, since 60% or more of the quarter remains; must enroll at new school.

Homework Policy
If your child is absent for two (2) or more days, you may call the school and request assignments and books. However, you must allow the teachers 24-hours to assign the homework and have it in the office for you. Once you have requested homework, please be sure to pick it up.
Textbook Policy
Students are issued textbooks and are allowed to borrow library books as part of the curriculum. If a student damages or loses texbooks or library books, he/she shall be responsible for the replacement cost (Hawaii Revised Statutes 298-5) indicated on the bill that is issued by the teacher or library staff.

• Parents confer with child’s teachers to modify the action plan.
• If the action plan that had been developed and modified does not work, the teacher or parent may bring the matter to the Student Services Coordinator for a more comprehensive evaluation.
• The Student Services Coordinator will convene a meeting of the Student Support Team (SST), which is made up of a teacher, parent, administrator, and special education representative.
• This SST will determine if a comprehensive evaluation is needed.
• If needed, comprehensive testing will take place.
• After testing, a meeting is held to discuss the results of the test and determine if special education criteria have been met.
